A)
CONSULTANCY
1)
Items provided by KEITH CHADWICK ASSOCIATES:
A quotation for provision Consultancy will be provided by KEITH CHADWICK
ASSOCIATES.
Invoice on completion of consultancy (or at pre-arranged intervals as
agreed).
2) Items provided by the Customer:
Letter of contract/written contract on terms and conditions of the
consultancy required.
Agreed schedule of reporting and clear definition of area of work.
Payment within 28 days of receipt of invoice/s.
B)
TRAINING COURSES
IN-HOUSE TRAINING COURSES
The
following are the terms and conditions of all contracts for the
provision of training courses:
1)
Items provided by CBT-CENTRE (Manchester Centre for Cognitive Behaviour
Therapy):
A
quotation for provision of trainers, participants manual, agreed course
programme, travel and accommodation (VAT liable).
Experienced and appropriate trainers or Facilitators.
Course
Programme (agreed with customer)
Copy
of participants training manual for customer to copy for each
participant (if appropriate).
Certificates of Attendance (Master copy) to be copied by the customer
for each participant.
Invoice on completion of training course (to be paid within 28 days of
receipt).
2)
Items provided by customer:
Appropriate training or meeting venue
Refreshments as required/agreed with trainers/facilitators.
3) If
the customer cancels the training course:
Less
than 14 days prior to the first day of training will result in payment
of the full agreed fee (inclusive of travel costs).
Less
than 28 - 15 days prior to the first day of training will result in
payment of 50% of the fee.
C) INDIVIDUAL/TEAM COACHING/DEBRIEFING
1)

As Associate Members of the British Association for Counselling and
Psychotherapy (BACP) we are bound by its Ethical Framework for Good
Practice in Counselling and Psychotherapy and subject to the
Professional Conduct Procedure for the time being in force.

2)
Cancelling sessions and changing time and/or day of sessions:
One week's notice is required to cancel a session or to change the time
or day of a session.
3) Length of sessions:
To be agreed.
4) Payment of sessions:
Individuals: payment to be made at the end of each session. Receipt
provided.
Corporate/Insurers: Terms as agreed.
D)
COMPLAINTS PROCEDURE
Any
complaints concerning services delivered by Keith Chadwick Associates
should in the first instance be in writing and addressed to the
Director. On receipt of complaints a letter of confirmation will be sent
and the complaint will be forwarded to the internal verifier for
consideration. A response to the complaint will be forwarded within 21
days of receipt.

1 ACCEPTANCE OF TERMS

notifying you of changes or updates to the Website/Services.
